Please fill out and submit our short COVID-19 consent and questionnaire prior to every appointment. We must have a record of this signed in order to see the patient.

We are happy to announce we are ready to welcome you back!

We have implemented an even more meticulous cleaning regime in all areas of our office in order to continue providing the best care possible:

  • Frequently touched surfaces will be constantly wiped down with sanitizing wipes
  • Our clinical staff continues their usual use of proper PPE which include masks, glasses, and gloves when seeing patients and will be using a shield and gown as well
  • Patient chairs continue to be completely wiped down and cleaned with proper disinfectants after each patient according to CDC protocols
  • Lobby chairs, books, and magazines have been removed
  • Multiple hand sanitizer bottles will be available for use throughout the office
  • Front desk area will have sneeze guards

In preparation for your appointment, here are some
useful reminders:

  • To protect our patients and staff, we are allowing only the patient to come inside the office.
  • When you arrive, please send us a text message at 832-680-0940 with the patient's name. The number will also be on the door.
  • Please fill out and submit our short COVID-19 consent and questionnaire prior to your appointment. You may also print them and bring paper copies to your appointment. We must have a record of these signed in order to see the patient.
  • All patients will have their temperature taken and will be screened for symptoms. If temperature is higher than 100.4, patient will need to be rescheduled. Patient shouldn’t eat/drink any very cold/hot foods/drinks 30 minutes prior to appointment, as this could give us an incorrect temperature reading.
  • Patient should brush and floss at home prior to their appointment.
  • Patients may be asked to rinse with hydrogen peroxide.
  • Patients should wear a mask when coming to the appointment if they have one.
  • If you need to make a payment during your visit, please call the office at 713-721-2426 to pay over the phone with a credit card or pay online under the "Payments" tab. If you are looking to make a payment with cash, please send it with the patient in an envelope.
  • As a precaution, our office has decided to minimize any possible exposure for our patients and staff by limiting the number of patients we see per day. We are specifically reserving the appointment time for you, which means rescheduling is going to be very difficult. If you need to reschedule, please let us know immediately and we will try to accommodate you. However, be advised that the next available appointment may be a couple of weeks away.
